Notifying the Public Rights Under Title VI & Title II

WHITE  COUNTY COUNCIL ON AGING/ PUBLIC TRANSIT

White County Public Transit operates its programs and services without regard to race, color or national origin in accordance with the Title VI of the Civil Rights. Any person who believes he or she as been aggrieved by any unlawful discriminatory practice under Title VI may file a complaint with White County Public Transit/ White County Council of Aging.

White County Council on Aging, P.O. Box 421, Monticello, IN 47960. 
Verbal complaints are accepted and transcribed by the Executive Director.  To make a verbal complaint, 574-583-9119 and ask for the Executive Director.
 
White County Council on Aging investigates complaints received no more than 60 days after the alleged incident. White County Council on Aging will process complaints that are complete. Once the complaint is received, White County Council on Aging will review it and the complainant will receive an acknowledgement letter informing them whether the complaint will be investigated by White County Council on Aging.

White County Council on Aging has up to fifteen days to investigate the complaint. If more information is needed to resolve the case, the White County Council on Aging may contact the complainant. The complainant has fifteen days from the date of the letter to send requested information to the investigator assigned to the case.

If White County Council on Aging’s investigator is not contacted by the complainant or does not receive the additional Information within fifteen days, White County Council on Aging can administratively close the case. A case can be administratively closed also if the complainant no longer wishes to pursue their case.

After the investigator reviews the complaint, one of two letters will be issued to the complainant: a closure letter or a letter of finding (LOF). A closure letter summarizes the allegations and states that there was not a Title VI violation and that the case will be closed.

A LOF summarizes the allegations and the interviews regarding the alleged incident, and explains whether any disciplinary action, additional training of the staff member, or other action will occur. If the complainant wishes to appeal the decision, she/he has ten days after the date of the letter or the LOF to do so. The appeal can be made within 15 calendar days after receipt of the response to the City of Monticello ADA Coordinator.
 
Within 15 days after receipt of the appeal, the Executive Director or his/her designee and the City of Monticello ADA Coordinator will meet with the complainant to discuss the complaint and possible resolutions. Within 15 days after the meeting the Executive Director or his/her designee and the City of Monticello ADA Coordinator will respond in writing, and, where appropriate, in a format accessible to the complainant, with a final resolution of the complaint.
